The MDS Nurse provides leadership and clinical assistance to all facility personnel to ensure facility compliance with Nursing Care Standards as applied to Resource Utilization Groups. The MDS Nurse assume responsibility for accurate completion of MDS 3.0 and accurate documentation to support MDS assessments.
RESPONSIBILITIES:
May participate in the admission process of prospective residents as it relates to their nursing need and appropriateness of placement
Assesses resident?s potential eligibility for Medicare coverage. Initiates Medicare Certification and recertification for physician signature as needed.
Monitor assessment schedules and nursing documentation (C.N.A. documentation, nurses notes, care plans) to ensure accuracy and timely submission.
Checks MDS and PCC Diagnosis dashboard daily, add Active Diagnosis as needed in PCC.
Ensure medical record supports assessments. Update medical record (Care Plans, Notes, etc.) as necessary. Updates care plan for triggers identified in MDS assessments which may include actual/potential problems and active diagnosis as needed.
Checks all MDS scheduled in the software for completion and alert other discipline as needed.
Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations
Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
Works closely with the interdisciplinary team.
Utilizing the RAI, MDS 3.0, QM, CBQM, and MBP manuals to inform activities and assessments.
Provide formal and informal MDS education as needed.
Act in a supporting role to the director of the facility.
All other duties as assigned.
QUALIFICATIONS / REQUIREMENTS:
Experience:
2-3 years Long Term Care Required. Supervisory/MDS Coordinator Experience Preferred.
Education:
Graduate of an accredited School of Nursing, BSN Preferred
Licensure/Certifications: Valid and current NYS Registered Nurse and MDS AANAC Certification Preferred
