Senior Social Worker-Community Living Center

SummaryThe Senior Social Worker will actively manage a case load of complex Veterans within the secured Dementia unit. Veterans have significant challenges and require specialized social work interventions due to cognitive impairments. Will serve on multidisciplinary teams working with patients dealing with issues including end of life - memory and cognitive impairments - disabilities and chronic conditions.
